Job Summary
This is a full-time remote opportunity with 50-75% travel in Cumberland and surrounding counties. The schedule is Monday - Friday, reputed company-5pm EST. As a Field Case Manager RN, you will be responsible for:
- Conducting face-to-face visits in members' homes, utilizing comprehensive assessment tools for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Program (D-SNP/FIDE)
- Developing, implementing, supporting, and promoting health service strategies, tactics, policies, and programs that drive the delivery of quality healthcare to our members
- Coordinating and collaborating care with members, primary care physicians, and other care team participants
- Scheduling and attending interdisciplinary meetings and advocating on members' behalf to ensure proper and safe discharge with appropriate services in reputed company
- Developing care plans and authorizing services in a cost-effective manner reputed company the MLTSS/FIDE benefit
- Documenting accurately and timely in members' electronic health records
- Mentoring new hires once a level of proficiency has been attained in their role
Key Responsibilities
Member-Centric Care
- Conduct comprehensive assessments of members' medical, behavioral, and psychosocial needs

- Collaborate with members, families, and care teams to ensure seamless care coordination
Care Planning and Coordination

Clinical Judgment and Critical Thinking

Requirements
Education and Licensure
- Active and unrestricted RN license in New Jersey
- Minimum of an Associate's degree in Nursing required
Experience and Skills
- 3+ years of clinical practice experience in a hospital setting, alternative care setting, or home health
- Must reside in reputed company or an adjacent county
- Willing and able to travel 50-75% in reputed company and surrounding counties
- Successful completion of the NJ Choice certification is a contingency of employment
