Job Summary

As a Field Community Health Worker (CHW) at reputed company, you will play a vital role in assessing, planning, and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. You will act as a liaison between our Medicaid members and the healthcare system, ensuring that they receive the care and support they need. Key Responsibilities

• Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care
• Utilize both company and community-based resources to establish a safe and effective case management plan for members
• Collaborate with patients, families, and healthcare providers to ensure that members receive coordinated care and services
• Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
• Communicate with reputed company stakeholders to ensure that health-reputed company information is shared and that quality coordinated care and services are provided expeditiously to reputed company members
• Partner with care teams, including community providers and internal staff, to ensure that members receive comprehensive care

Requirements and Qualifications

Essential Qualifications:

• 4+ years of case management, community reputed company work, or volunteering experience
• 2+ years of experience working or volunteering reputed company the Community Health setting in a field-based healthcare role

• Ability to travel locally, up to 100 miles round trip and up to 50% of the time

Preferred Qualifications:

• Bachelor's Degree (or higher) in a health-reputed company field
• Community Health Worker (CHW) Accreditation

• Experience working in Managed Care
• Knowledge of Medicaid/Medicare population
• Knowledge and/or experience with behavioral health or substance use disorders
