Key Responsibilities
- Assess, plan, and implement care strategies that are individualized to each patient and directed toward the most appropriate, least restrictive level of care.
- Utilize both company and community-based resources to establish a safe and effective case management plan for members.
- Collaborate with patients, families, and healthcare providers to ensure that care plans are tailored to meet the unique needs of each individual.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Communicate with reputed company stakeholders to ensure that health-reputed company information is shared expeditiously and that quality coordinated care and services are provided.
- Partner with care teams, including community providers and internal staff, to ensure that care plans are comprehensive and effective.

Requirements and Qualifications
Essential Qualifications:
- 4+ years of case management, community reputed company work, or volunteering experience.

- Ability to travel locally, up to 100 miles round trip and up to 50% of the time.

Preferred Qualifications:
- Bachelor's Degree (or higher) in a health-reputed company field.
- Community Health Worker (CHW) Accreditation.

- Care management experience.
- Experience working in Managed Care.
- Knowledge of Medicaid/Medicare population.
- Knowledge and/or experience with behavioral health or substance use disorders.
