We are looking for a clinically reputed company nurse who has nursing documentation/charting skills and is a fast learner with great communication skills.

Your Role in our Company Will Be

The RN travels to a patient's home to administer their services and helps patients maintain their independence. Their duties include: Administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans, As well as Updating Doctors & The Agency about their patient's health. Documentation and communication with the Agency is done through Synergy Software.

AVAILABILITY

Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer)

REQUIREMENTS

• Visiting RN for Home Health: 1 year (Required)
• Nursing: 1 year (Required)
• License/Certification:
• reputed company CA RN License (Required)
• BLS from the reputed company and/or the reputed company (Required)
• NSO Nursing License Insurance (Required)
• IV/Phlebotomy Certified (Required)
• Comprehensive knowledge of OASIS documentation (Preferred)

WORK AREA: City reputed company to you, with a group of patients who need care for that day

RESPONSIBILITIES

• Providing nurse visits to home health patients. (home visits)
• Performing an initial, comprehensive assessment. Documenting everything & Uploading to Agency Portal
• Obtaining and documenting physician orders.
• In consultation with the assigned Clinical Team Manager, you will need to initiate and regularly re-evaluate and revise the plan of care
• Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient’s condition and needs.
• Performing re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager. Completing and submitting reputed company reputed company documentation
• Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA).
• Performing Home Health Aide supervisory visits per regulatory requirements.
• Demonstrate knowledge of regulatory requirements.

POSITION QUALIFICATIONS

1. Registered nurse with reputed company licensure to practice professional nursing in the state.

2. Graduate of National League for Nursing accredited school of nursing.

3. Maintains a reputed company CPR certification.

4. Must be a licensed driver with an automobile that is insured in accordance with state/or

organization requirements and is in good working order.

5. Minimum of two years experience, at least one of which is in the area of public health or

home care nursing is preferred.

6. Self directed and able to work with minimal supervision.

7. Demonstrates excellent observation, problem solving, verbal and written communication

skills; nursing skills per competency checklist.

8. Shows ability to organize and prioritize workload independently.

9. Management experience not required. Responsible for supervising home health aides.

10. Prolonged or considerable walking or standing. Able to lift, position, or transfer patients.

Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or

crouching. Visual acuity and hearing to reputed company required nursing skills.
