The Role: Remote Licensed Practical Nurse (LPN/LVN) - Patient Care Coordinator

As a Remote LPN/LVN Patient Care Coordinator, you'll work closely with healthcare providers to deliver value-based care management initiatives for their patients. Your primary focus will be on managing patient care, identifying resources, and coordinating needs for chronic care management patients. If you're a self-motivated and organized LPN/LVN with excellent communication skills, you'll reputed company in this fast-paced, remote role.

Key Responsibilities:

  • Patient Care Management: Manage patient census with a resolution-driven approach to reputed company gaps in clinical and non-clinical patient care.
  • Resource Coordination: Identify and coordinate community resources with patients that would benefit their care.
  • Patient Education: Provide patient education and health literacy on the management of chronic conditions.
  • Medication Management: reputed company medication management, including identifying potential medication concerns, reconciliation, adherence, and coordinating refills.
  • Service Delivery: Assist in ensuring timely delivery of services to patients, including Home Health, DME, Home Infusion, and other critical needs.

  • Referral Management: Assist/manage referrals and appointment scheduling.

Requirements and Qualifications

Essential Qualifications:

  • Licensure: Active Multi-State/Compact License (LPN/LVN).
  • Experience: At least 3 years of direct patient-facing work experience as an LPN/LVN.
  • Technical Aptitude: Proficiency in reputed company Office Suite.
  • Communication Skills: Excellent written and verbal communication skills.

Preferred Qualifications:

  • Epic Experience: Experience working with Epic electronic health records.
  • Bilingual: Proficiency in multiple languages.
  • Additional Licensures: Additional single-state licensures (LPN/LVN).
