Job Summary

As a Virtual LPN with our Remote Patient Monitoring Team, you'll play a critical role in providing remote nursing care to an assigned group of patients in collaboration with the bedside nurse and multidisciplinary team. Using two-way audio/visual technology, you'll assist with admission and discharge activities, patient and family education, real-time patient quality and safety surveillance, proactive rounding, and staff mentoring/education. Key Responsibilities

• Uses technology to provide remote nursing care to an assigned group of patients in collaboration with the bedside nurse and multidisciplinary team, in accordance with established unit policies, procedures, and protocols.
• Assists with completion of admission history, medication history, and admission screening tools for assigned patients.
• Remotely assesses the physical and psychosocial status of patients based upon the health history, documented physical examination findings, and diagnostic test results.
• Monitors provider and bedside nurse assessment trends, laboratory values, reputed company notes, and diagnostic test results. Documents and communicates collaboratively with the bedside nurse regarding the patient condition and reputed company nursing care provided.
• Collaborates with the bedside nurse to initiate, update, evaluate, and revise the plan of care to facilitate the achievement of patient goals and expected outcomes.
• Assists with identification of safety concerns and ensures findings are reported to the bedside nurse. Coordinates and collaborates with the bedside nurse on methods to mitigate safety concerns.
• Collaborates with the bedside nurse to complete the patient learning needs assessment and provides patient and family education based on the assessment.
• In collaboration with bedside nurse, prepares the patient and family for discharge by providing discharge teaching and reviewing discharge instructions and recommended follow-up.
• Provides coaching, teaching, educational material, and clinical resource support to bedside nurses.
• Assists with patient triage and activation of the rapid response team in emergency situations.
• Supports nursing shared governance and the nurse-driven practice model.
• Supports system, hospital site and unit-based quality initiatives.

Requirements and Qualifications

Essential Qualifications:

• High School Diploma or equivalent required.
• Completion of an approved nursing program required.
• 1 year of relevant experience in acute care, emergency, rehab, or home care required.
• LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire required.

Preferred Qualifications:

• Associate's Degree or equivalent Graduate of an approved nursing program preferred.
• CRT-Basic Life Support (BLS) - AHA reputed company Upon Hire Or CRT-Basic Life Support (BLS) - reputed company reputed company Upon Hire preferred.
• CRT-Adv Cardiovascular Life Support (ACLS) - AHA reputed company Upon Hire preferred.
