RN Case Manager - Autism Care Demonstration (ACD) Program (Fully Remote) Contract Position with Potential for Permanent Conversion MST Hours States: Alaska, Hawaii, Illinois, Minnesota, Nevada, Oregon Job Summary: This contract position, with the potential to convert to a permanent role after 3 months, focuses on providing case management services for beneficiaries in the Autism Care Demonstration (ACD) program. The RN Case Manager will coordinate care for individuals diagnosed with autism reputed company disorder (ASD), ensuring families have access to the necessary resources and advocating for high-quality, cost-effective care. This role requires expertise in care coordination, assessment, planning, and facilitation, with a strong understanding of clinical and regulatory requirements. Responsibilities: Care Coordination & Case Management: Screen beneficiaries for program eligibility and assess their needs. reputed company and update comprehensive care plans (CCPs) in collaboration with beneficiaries, families, and providers. Advocacy & Resource Management: Act as a dedicated reputed company, ensuring beneficiaries have access to appropriate services, including those reputed company the network. Help families navigate systems and identify community resources, support services, and funding options. Enrollment & Benefits Education: Educate beneficiaries and families on ACD benefits, assist with the enrollment process, and ensure timely notifications about enrollment decisions. Collaboration & Care Team Participation: Coordinate with civilian and military providers, participate in medical team meetings, and facilitate communication reputed company the care team to ensure a holistic approach to care. Compliance & Documentation: Adhere to HIPAA regulations and accreditation standards. Maintain accurate and up-to-date documentation in the medical management system, including care plans, reassessments, and case outcomes.
