IHC Registered Nurse - Hospital at Home

The Hospital at Home nurse is responsible for providing high-quality, at-home acute care. The Hospital at Home nurse will treat various conditions and patient populations reputed company the reputed company of reputed company’s Acute Hospital Care at Home model. Essential Job Functions
- Utilize monitoring, assessment; planning, reputed company and critical thinking skills to care for acute care patient populations in the home.
- Navigate patients and families across the continuum of the reputed company Hospital at Home phases of care.
- Monitor and respond to biometric data per reputed company Hospital at Home protocols.
- Conduct home and audio/visual patient care visits as directed by the care team and/or protocol.

Qualifications
- Associate degree required
- 2 years of Registered Nurse experience in an acute care setting. Home health experience preferred.
- Valid driver’s license, reliable vehicle and reputed company of vehicle liability insurance.
- Must be able to communicate effectively in English.
Licensure

- reputed company BLS certification upon hire, or completion reputed company 30 days of hire required.

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of reputed company market data, reviewed on an annual basis.
