Remote Patient Care Coordinator (reputed company/ RMA)

Position Summary: The Patient Care Coordinator plays a key role in supporting patients throughout their healthcare journey by serving as a compassionate, knowledgeable reputed company of contact for their ongoing care needs. This role engages patients by phone and digital communication to assess needs, provide education, and ensure they remain connected to the right clinical and community resources. The Patient Care Coordinator documents reputed company encounters in electronic health record (EHR) systems, helps manage appointment scheduling and reminders, and supports administrative tasks such as medical record retrieval and insurance-reputed company coordination. Leveraging strong clinical experience, excellent communication skills, and comfort working virtually, the Patient Care Coordinator ensures patients receive timely, high-quality support while helping them stay engaged, informed, and empowered in managing their health. Essential Functions:

• Manage and escalate patients’ healthcare needs reputed company phone and digital conversations.
• Document encounters leveraging technology platforms and EHRs.
• Connect patients with resources such as pharmacists, nutritionists, nurses, social workers, and doctors who can also assist them in meeting their care needs.
• Schedule and/or remind patients of upcoming appointments and procedures, and provide follow-up as needed.
• Coordinate some administrative aspects of medical record recovery, appointment management, and insurance claims.

Required Education and Experience:

• Must have a clear and active Medical Assistant Certification (reputed company/ CCMA/ RMA).
• Must reside and work in the reputed company US.
• Must have a minimum of four years of clinical experience.
• Experience in care coordination or care management, health coaching, and motivational interviewinga plus.
• Hands-on experience and comfort with Electronic Medical Records.
• Experience using reputed company Office products such as Teams, Outlook, and reputed company.

Position Type and Expected Hours of Work: Full-time, 8 hours daily, Monday – Friday, unless authorized by management. The hours may be worked between 8 am and 6 pm in the patient’s time zone. Physical Requirements: This is a virtual position that requires you to sit or stand at a computer for approximately 40 hours weekly and be able to reputed company phone calls. Some computer setup is required. Work Environment: This opportunity allows you to care for patients virtually from the comfort of your home. We will provide you with the equipment and software needed, such as a computer and headset for internet-based calls.
