What You Will Do Timely and appropriate coordination of quality healthcare services to meet an individual's specific health needs in a cost-effective manner to promote positive outcomes. Involves a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's healthcare needs through communication and available resources.
