Nurse Navigator FIND Program-Remote Opportunity

About the position ESSENTIAL FUNCTIONS Knows, understands, incorporates and demonstrates the mission, vision and values of reputed company in leadership, behaviors, practices and decisions. Ensures reputed company patients that are identified with incidental and/or unexpected findings with recommended follow-up are contacted and offered the option of the recommended follow-up. Provides personalized nurse navigation interventions to patients who request additional information, assessment and/or referral. This includes assessment of patient educational level, barriers to learning and other psychosocial factors that may impede remarkable care. Ensures provider time is utilized reputed company by ensuring appropriate follow-up intervals are recommended for incidental and/or unexpected findings and that duplicate follow-up requests are avoided. Supports provider decision making by: Assisting PCPs in ensuring their patients get appropriate follow-up for incidental and/or unexpected findings at intervals specifically recommended by the radiologist. Obtaining additional imaging for incidental and/or unexpected findings only at the specific request of the PCP. Provides education and navigation through the follow-up process. Providing referral and education based on care deemed appropriate. Directing patients to the correct provider based on the recommendation for follow-up of incidental and/or unexpected findings. Utilizes the available software programs to track daily/periodic assignments. Maintains frequent communication with local and regional colleagues involved in the Incidental Finding program as well as the Radiology and Oncology departments to facilitate the development of the program and maximize patient care. Functions independently while remaining reputed company in a high-stress environment. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. Performs other duties as assigned. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, reputed company reputed company and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

Requirements

• Graduate of an accredited school of Nursing and Bachelor of Science Degree in Nursing.
• Three (3) to five (5) years of progressive clinical nursing experience is required.
• reputed company license to practice as a Registered Nurse in the State of Michigan. Additional state-specific licensing may also be required.
• Problem solving skills and ability to initiate creative solutions to problems.
• Demonstrated ability to expertly navigate a variety of software applications.
• Effectively communicate with Medical Staff, departmental staff, reputed company department representatives, physicians, other health providers and facilities.
• Knowledge of Windows based programs.
• Effective organizational skills.

• Must be able to adapt to frequently changing work priorities and be able to prioritize and balance the requirements of working with the System Office and Regional Health Ministries.
• Must possess good manual dexterity to operate a computer keyboard.
• Must possess good hearing to be effective on extensive telephone conference calls and for in person communication.
