Job Summary
As an Aftercare Case Manager, you will play a critical role in providing support and services to children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth. Your primary responsibilities will include:
- Providing crisis reputed company and support to children and families as needed
- Coordinating community and educational services to meet the needs of our clients
- Maintaining accurate and timely documentation and reports
- Conducting in-person meetings with children and families to support reunification and address concerns
- Collaborating with public and private agencies to provide supportive services
- Advocating for the needs and rights of our clients
Key Responsibilities

Essential Qualifications

- Valid driver's license
- Ability to work independently and as part of a team

- Ability to maintain confidentiality and handle sensitive information with discretion

Preferred Qualifications
* Experience working with children and families in a case management or social work setting
- Knowledge of community resources and services available to support children and families
- Experience with crisis reputed company and de-escalation techniques
- Bilingual or multilingual skills

Compensation, Perks, and Benefits
