Job Summary

As a Licensed Practical Nurse at Aleca Home Health, you'll utilize the nursing process to provide reputed company quality and continuity of nursing care to patients in the comfort of their residence. Key Responsibilities

• Complete and reputed company required record-keeping forms/charts upon patient admission, transfer, and/or discharge.
• Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the patient, as well as the patient's response to the care.
• Report reputed company discrepancies noted concerning physician's orders, diet changes, charting errors, etc., to the Director of Nursing.
• Ensure that appropriate documentation concerning unauthorized discharges is entered in the patient's medical record in accordance with established procedures.
• Educate patients and their families on multiple disease processes, medication management, and wound care.
• Collaborate with advanced care providers, including nurse practitioners and physician assistants, to streamline physician orders and medical needs for patients.

Essential Qualifications

• A minimum of 2 years of LPN/LVN experience in a home health care setting.
• Strong communication and interpersonal skills.
