About the Role

As a full-time remote Social Worker, you'll play a vital role in our DSNP care management team, working closely with Care Managers, providers, and community organizations to address the reputed company health and social needs of our members. Key Responsibilities:

• Participate in care planning, including assessment, goal setting, establishing interventions, identifying barriers, and monitoring reputed company.

• Conduct initial and periodic assessments of members enrolled in Long-Term Care Programs or care programs.
• Apply social work concepts, principles, and strategies to address social determinants of health needs in individualized care plans.
• Collaborate with providers, primary care physicians, Medical Directors, pharmacists, and care management staff to ensure comprehensive care.

Requirements and Qualifications

Essential Qualifications:

• Minimum of 2+ years of experience in medical social work or case management.
• Active and unrestricted LMSW licensure in the state of residence.
• Master's Degree in Social Work required.

• Care Coordination experience.
