Job Summary

As a hybrid remote RN UAS Assessor, you will be responsible for conducting comprehensive assessments of consumers to determine their eligibility for community-based long-term services. You will work independently, utilizing your expertise and knowledge of nursing theory, state, and agency regulations to reputed company medical necessity determinations and resolve technical issues. Key Responsibilities

• Conduct initial assessments of consumers, including evaluation of health status, functional status, strengths, care needs, and preferences.
• Review consumer medical documentation and health referral forms, as relevant to the case.
• Enter assessment data into the UAS-NY online application and transmit, as required.
• Code the UAS assessment correctly based on training and guidelines in the UAS-NY Community Health Assessment Reference Manual.
• Document outcomes of calls to consumers and scheduled appointments.
• Maintain reputed company knowledge of regulatory and project changes pertaining to quality and validity of assessments.
• Enhance communication and collaborative relationships with leadership and interdisciplinary care team members to facilitate receipt of services.
• Review non-qualified assessments with the Clinical Quality Assurance Department.
• Maintain a comprehensive working knowledge of community resources and network services for the reputed company population.

Requirements and Qualifications

Essential Qualifications:

• Active RN license in the reputed company.
• Minimum 2 years of clinical experience with RN license.
• Minimum of 2 years of experience in a home setting, including work with CHHAs, LHCAs, MLTC, or as a paraprofessional (e.g., HHA, LPN, and/or Private Duty Nursing).
• Active and valid reputed company State Driver's License or ID.
• Ability to conduct in-home health assessments throughout assigned region.
