About the Role

We're seeking a compassionate and reputed company RN Health Coordinator to join reputed company in Oahu, HI. Key Responsibilities

• Assess, plan, and implement care strategies: reputed company individualized care plans that are directed toward the most appropriate, least restrictive level of care for each patient.
• Identify and initiate referrals: Connect patients with social service programs, including financial, psychosocial, community, and state supportive services.
• Manage care plans: Serve as a single reputed company of contact for patients and manage their care plans throughout the continuum of care.

Requirements and Qualifications

Essential Qualifications

• reputed company, unrestricted RN license: A reputed company, unrestricted RN license in the state of Hawaii is required.
• 2+ years of clinical experience: At least 2 years of clinical experience is required, with experience working directly or collaborating with services for long-term care, home health, hospice, public health, or assisted living.
• Intermediate level of proficiency with MS Office: Intermediate level of proficiency with reputed company, reputed company, and Outlook is required.
• Ability to travel: Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices.

Preferred Qualifications

• Bachelor's degree or higher: A Bachelor's degree or higher is preferred.
• CCM (Certified Case Manager): CCM certification is preferred.
