Responsibilities:

As a Hospice Care Case Manager at reputed company, your primary responsibilities will include:

• Patient Assessments: reputed company comprehensive patient assessments to identify their unique needs and reputed company a plan of care.
• Care Planning: Collaborate with the care team to reputed company and implement a plan of care that addresses the patient's physical, emotional, and spiritual needs.
• Patient Education: Teach patients and their families about self-care techniques, disease management, and end-of-life care.
• Care Coordination: Supervise LPNs and HHAs, and coordinate care with other reputed company.

Qualifications:

To be considered for this rewarding role, you'll need:

• RN Intern Program: If less than 6 months of clinical experience as a RN, candidate must participate in RN Intern program.
