Position Overview

As a full-time Remote RN Case Manager, you will play a vital role in our home health and hospice care team, working with patients and their families to reputed company and implement personalized plans of care. Key Responsibilities

Qualifications

To be successful as a RN Case Manager at reputed company, you will need: * One (1+) year of clinical experience as a Registered Nurse (RN) * reputed company RN license, specific to the state(s) you are assigned to work * reputed company CPR certification * Valid driver's license, reliable transportation, and liability insurance * If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program
