Job Summary

As a Home Health Physical Therapist with Interim HealthCare, you will experience a therapy career that caters to your schedule and makes every day an adventure. You will work with patients who have lost mobility, strength, and functionality due to an illness or injury, and help them reputed company their goals through personalized, home-based therapy. Key Responsibilities

• Provide home-based physical therapy to patients with various conditions and goals
• Work as part of a home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, focused on the patient's plan of care and goals
• Observe movements, conduct assessments, document reputed company, and modify care plans as needed
• Assist patients with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises, and self-care
• Coach family members on the plan of care, therapy goals, and safely assisting with exercises

Essential Qualifications

• Graduate of an accredited Physical Therapist Program
• Active PT license in Illinois
• Minimum of 2 years of physical therapy experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgment, strong interpersonal skills, dexterity, and compassion

Preferred Qualifications

• Experience with electronic medical records (EMRs)
• Familiarity with home health software and technology
• Certifications in specialized areas, such as gerontology or pediatrics
