Key Responsibilities

• Serve as the SNBC (Special Needs Basic Care) Coordinator, ensuring access to healthcare services, providing advocacy, and helping clients navigate complex healthcare delivery systems.
• Conduct comprehensive nursing assessments in assigned programs, providing information and referrals to health and human services programs through home visits and community settings.
• Provide ongoing case management for clients, maintaining case notes and client files to meet State and Federal requirements.
• Deliver health teaching and education to promote positive health outcomes and prevent illness, investigating disease and other health events as needed.
• Foster collaborative relationships with the public, community organizations, and other agencies to promote health and well-being.
• Respond to disasters and assist communities in recovery, recognizing deviations from the norm that might indicate an emergency.

Essential Qualifications

To be successful in this role, you will need:

• A current Registered Nurse (RN) licensure in the state of Minnesota.
• A thorough understanding of public health nursing theory and practices, with the ability to apply principles to communities, families, and individuals.
• Excellent communication and interpersonal skills, with the ability to establish and maintain effective relationships with clients, agency staff, and other stakeholders.
• The ability to work independently, making informed decisions and utilizing community resources effectively.
• A valid driver's license, with the ability to travel to various community settings.

Preferred Qualifications

While not required, the following qualifications are highly preferred:

• A Bachelor of Science in Nursing (BSN) degree or Public Health Nursing (PHN) certification.
• A minimum of 1-2 years of experience in public health nursing or a related field.
