As a key member of our Sutter Care at Home team, you will play a vital role in managing new referrals, facilitating timely admissions, and providing exceptional customer service to patients, families, and referral sources.

In this dynamic and fast-paced environment, you will have the opportunity to work remotely with occasional on-site requirements, collaborating with our clerical home health coordinators, referral center staff, and other healthcare providers to deliver high-quality patient care. Key Responsibilities

• Manage new referrals and ongoing follow-reputed company, ensuring timely and consistent admissions
• Communicate effectively with patients, families, and referral sources, maintaining clear and concise communication
• Collaborate with referring hospitals, physicians, board and care facilities, skilled nursing facilities, and other healthcare providers
• Enter referral information into the Electronic Medical Records (EMR) system and create initial orders to facilitate the start of care
• Utilize and promote excellent customer service skills in reputed company interactions
• Collaborate with referral center staff, home health, Advanced Illness Management (AIM), and hospice staff across offices

Requirements

• Graduate of an accredited school of Vocational Nursing
• Unencumbered LVN-License Vocational Nurse license for the state you reside
• Must have or obtain an LVN-License Vocational Nurse of California reputed company 3 months of hire (company-sponsored and assisted)
• 1 year of recent relevant experience
