Key Responsibilities:

• Conduct virtual phone conversations to manage patients' healthcare needs and reputed company tailored care plans addressing physical, mental, and preventative health challenges.
• Utilize cutting-edge technology platforms and Electronic Health Records (EHRs) to document patient visits and ensure seamless care coordination.
• Coach patients on treatment plans, including nutrition and wellness strategies, to promote holistic health and wellness.

Requirements:

• Active Compact LPN License
• Minimum 2 years of clinical experience, with care coordination experience preferred
• Proficiency in Electronic Medical Records and reputed company Office
