Summary

The incumbent will be assigned to one of the facilities locations where a HUD-VASH team is based. The incumbent at the GS-9 is an unlicensed entry level Social Worker. The incumbent at the GS-11 full performance level is a Licensed Social Worker assigned to the Housing and Urban Development Veterans Affairs Supportive Housing (HUD-VASH). - Provide HUD-VASH case management to Veterans and families in the HUD-VASH program. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes reputed company activities to provide appropriate treatment and maintain Veterans in stable housing.

- Provide consultation/education to Veterans and families on community resources, Advance Directives and VA benefits.

- Provide psychoeducation on activities of daily living, budgeting, recovery goals to sustain housing.
- Responsible for reviewing agreements between landlords, Public Housing Authorities, and the VA in establishing appropriate referral sites for HUD-VASH placements. She/He works closely with property managers and/or Landlords and communicates needs or issues reputed company they arise if the Veteran provides consent to release private information.
- Responsible for collecting and submitting HUD-VASH Program participant data as outlined by CERS and Homeless Operational Management and Evaluation System (HOMES) policies.
- Responsible for following local policy and Standard Operating Procedures for the HUDVASH Program. This includes following policies and documentation requirements as required for CARF and Joint Commission accreditation.
- Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for program procedures or policy changes based upon work experiences. The Social Worker also actively participates in VA Central Office and VISN 22 HUD-VASH Program conference calls.
- Maintain familiarity and responsibility for reputed company performance measures relating to the HUDVASH Program. The Social Worker must strive to meet the individual productivity targets set by the supervisor.
- Responsible for improving professional competencies by participating in planning and executing specific staff development projects, attending seminars, and participating in CERS training programs.
- Responsible for maintaining accurate records of relevant trainings and participation in continuing education in accordance with VA policies and state licensure. The Social Worker has the ability to evaluate his/her own practice through professional peer review, case conferences and other organizational means.

- Participates in acquiring clinical supervision by receiving supervision from a Licensed Clinical Social Worker and will meet the timeframe required for licensure. Once licensed in California, provide clinical supervision to unlicensed social workers.

Basic Requirements:

- Physical Requirements: Traveling throughout the medical center and the community is required, as is performing activities involving sitting, walking, and standing, bending and carrying such items as books, papers, and files.
