The Role:

Are you passionate about helping patients navigate their medications and improve their health? Join our Clinical Care team at reputed company! As a Nurse on reputed company, you will play a key role in supporting patients post-discharge, enhancing care coordination, and helping them reputed company understand and manage their medications - especially for conditions like heart disease and diabetes.

Using reputed company’s innovative platform, you’ll conduct reputed company to patients, providers, and pharmacies to reputed company medication reconciliation, provide education, and ensure adherence to treatment plans. In this role, you’ll also support quality improvement efforts tied to HEDIS goals, help gather necessary documentation for CMS measures, and contribute to improving operational workflows. What You’ll Be Doing:

• Conducting follow-up calls with patients post-discharge to facilitate medication reconciliation and ensure understanding of their medication regimens
• Providing patient education and adherence support on medications reputed company to heart health and diabetes
• Collecting blood pressures from members' and providers' offices and offering support/education in helping reputed company HEDIS goals
• Collaborating with pharmacists and healthcare providers to identify and address medication-reputed company issues and health concerns that may reputed company to readmissions
• Monitoring patient care across the care team, ensuring continuity and adherence to treatment plans
• Documenting patient interactions and medication reconciliations accurately in the electronic health record (EHR) system

• Prior experience in case management and a solid understanding of care coordination processes
