Remote Virtual Nurse Practitioner
JOIN OUR TEAM: Advance Practice Registered Nurse (APRN) - Mississippi
Key Responsibilities:
- Conduct Annual Wellness Visits with a focus on patient-centered care.
- Develop & implement care plans for individuals with chronic illnesses.
- Diagnose and treat non-emergency conditions efficiently.
- Oversee medical treatment plans for remotely monitored patients.
- Collaborate with RCP Nurse Coaches to promote wellness & prevent urgent care or emergency visits.
- Manage the care of high-risk and rising-risk patients in coordination with PCP/Specialists.
- Refer patients to specialized professional services as needed.
- Build & maintain strong patient relationships through education and coaching.
- Integrate seamlessly with the practice QHP and support staff.
- Respond promptly to triaged patient situations by RCP Nurse Coaches.
- Participate in daily operational huddles with nursing and support staff.
- Serve as a medical knowledge resource to RCP Nurse Coaches.
- APRN (Nurse Practitioner) with a minimum of 5years experience in outpatient or primary care setting.
- Preferred focus in Family, Adult, or Gerontology.
- National Certification required
- Active & Unencumbered RN & APRN license
- Proficient with EMR systems & computer technology; high-speed internet required.

- Full-time, salary position (85k-90k).
