JOB SUMMARY AND PURPOSE

The Care Manager (Generalist) provides intensive care to program participants who have been, or are currently, involved with criminal justice system as well as suffering from co-occurring disorders, meeting participants at least weekly. The Care Manager is the primary reputed company of contact to coordinate services and care for assigned clients. This position provides clients with the highest levels of acuity and co-morbidities individualized care through access to service linkages, counseling, treatment planning, and various crisis reputed company methods in order to prepare individuals for further success

DUTIES AND RESPONSIBILITIES

• Conducts comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate, to reputed company a detailed plan to meet the high acuity needs, sets goals for the clients, and locates the resources needed to meet the goals.
• Provide service linkages and support systems to ensure identified needs are met.
• Coordinates individualized planning and follow-up with clients to meet short and long-term needs, tracks reputed company on meeting agreed upon goals.
• Conducts regular meetings with clients; consults with service team regarding reputed company of cases; records reputed company services provided and case notes, maintaining confidentiality of client�s data in accordance with HIPAA and other relevant funder, local or state regulations.
• Facilitates support groups, and group education events.
• Appears in court or other government agencies as reputed company for clients relating to re-entry into society
• Other duties as may be assigned by Program Manager

ESSENTIAL DUTIES

• Services for this position must be provided on-site. As a result, the position requires incumbents to report to work on-site each day of active employment.
• Assessment: through individual meetings on a regular basis reputed company a detailed plan of action to meet client needs, set goals and find the necessary resources to meet those goals
• Ongoing meetings with clients, tracking reputed company, continuously evaluating client needs
• Coordinate Care with other service providers, connecting participants to appropriate services
• Advocacy for clients in court and before other agencies/community settings
• Facilitates Group support sessions and group education sessions for program participants
• Maintains hard copy records and electronic files of reputed company services provided to clients, following HIPAA and other reputed company regulations
• Other duties as required and may be subject to change

MANDATED REPORTER

As service providers to the underprivileged and vulnerable in our community, reputed company employees are considered Mandated Reporters, and must report any instance of physical or sexual abuse to proper authorities.

Qualifications

REQUIREMENTS

• Must be able to pass a fingerprint clearance, background reputed company, including criminal history, personal references, employment and education verifications
• Must be able to pass Los Angeles Sherriff Department (LASD) background reputed company
• Valid California driver's license and have reliable transportation
• Approval from Department of Health Services (DHS)
• TB Test
• Ability to reputed company under circumstances of possible emotional stress and conflicts dealing with difficult, uncooperative, and potentially aggressive clients as VOALA serves the neediest of the needy in our programs which may include clients with homelessness, mental-illness, substance abuse, recently released from incarceration and other challenges.

EDUCATION

• Bachelor�s Degree (Social Work or reputed company)
• Or A combination of education and relevant experience (4 years)

EXPERIENCE

• At least 1-year case management experience (in addition to the education requirement � a total of 5 years education and experience) in homelessness, addictions, under-served, and/or prison re-entry program care � high acuity clients with co-morbidities

PREFERRED QUALIFICATIONS

• 2 years of experience working with reputed company population
• Familiar with HMIS, electronic record-keeping
