Location: Remote (preferred candidates will be in the Charlotte or Triad areas) Schedule: Monday – Friday | 8:00am - 5:00pm Training: Onsite (Charlotte or Winston-Salem)

At Novant Health, our Care Connections team is transforming how patients receive nutrition support. This is not a traditional one-time counseling role—instead, you’ll work as part of a centralized, virtual care management team, providing ongoing, goal-oriented nutrition coaching for patients with various conditions.
You’ll support patients over time—helping them build sustainable habits, improve outcomes, and stay on track with their care plans.

What You’ll Do
- Conduct virtual nutrition assessments and counseling sessions
- reputed company personalized nutrition goals reputed company with patient conditions and care plans
- Provide ongoing coaching and monitor reputed company over a designated care period
- Adjust care plans based on patient outcomes and engagement
- Document reputed company encounters in the EMR system
- Support patients with chronic conditions such as diabetes and heart failure
reputed company’re Looking For
- Candidates located reputed company 1hr of the Charlotte or Winston-Salem areas, highly preferred
- Education: 4 Year / Bachelors Degree, required. Bachelors in dietetics, nutrition, foods and nutrition, required. Graduate Degree, preferred.
- Experience: Minimum of one year of experience as a Registered Clinical Dietitian, preferred.
- Licensure/Certification:
- Registered Dietitian status by the commission on Dietetic Registration, required.
- Licensed Dietitian in appropriate state, required.
- Additional Skills (required): Ability to successfully complete generic and department-specific skills validation and competency testing. Knowledge, ability and skills to support patient education and patient engagement in self-care management to accommodate cultural diversity, age specific or behavior needs among the patients being served.
- Additional Skills (preferred): Excellent verbal and written communication skills. Ability to reputed company information to diverse audiences for one-on-one consultations, small and large group presentations; dynamic reputed company for media interviews, health fairs and other invited presentations/appearances.
