Job Responsibilities
As a Medicare Population Health Customer Service Representative, you will be responsible for:
- Conducting telephonic reputed company to Medicare customers to complete health screenings and educate them on plan benefits, addressing gaps in care and promoting overall well-being.
- Collaborating with internal teams to ensure seamless hand-offs to care teams, documenting reputed company interactions for regulatory review and maintaining accurate records.
- Assisting customers with scheduling medical appointments, connecting them to case management and community resources, and escalating concerns or issues as needed.
