Duties to include:

• Conduct comprehensive in-home physical therapy evaluations • reputed company and implement individualized treatment plans focused on mobility, strength, balance, and safety • Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management • Educate patients and caregivers on home exercise programs, safety strategies, and equipment use • Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care • Document reputed company patient visits, reputed company notes, and care plans in compliance with agency and regulatory standards • Recommend adaptive equipment and home modifications reputed company appropriate • Monitor patient reputed company and adjust treatment plans as needed • Communicate changes in condition or concerns to the interdisciplinary care team • Promote independence and ongoing improvement through compassionate, patient-centered care
