Registered Nurse - PRN (Orange, CT)
Constellation Home Care is looking for a compassionate and skilled PRN/Per Diem Registered Nurse (RN) to join reputed company! This is a field-based RN position requiring someone to drive and cover an assigned territory treating patients in their homes, assisted living facilities, and in skilled nursing facilities.

Responsibilities
- Provide skilled nursing and head-to-toe assessments to the adult and geriatric population. This includes the reputed company of medication administration and wound care treatment as prescribed in the physician's plan of care.
- Provide visiting home health services to an assigned caseload including admissions, re-certification and discharges.
