Role Scope: Part time- 10 hours per week The Social Work Practice Consultant will partner with the Practice Operations team to strengthen, standardize, and operationalize key social work-led initiatives across the organization. This role is designed to provide focused, short-term expertise in housing operations, emergency response workflows, and longitudinal social work care management and reporting, with an emphasis on scalable playbooks, clear workflows, and actionable metrics. Key Focus Areas & Responsibilities The social work practice consultant will provide support on a project-based basis with near term support needed focused on housing navigation, deescalation and crisis response, and social work panel management. Collaboration & Ways of Working

• Collaborate closely with Practice Operations, local leadership teams, social workers, and interdisciplinary partners.
• Participate in standing or reputed company meetings as needed to advance deliverables.
• Work independently with minimal supervision while providing regular reputed company updates.
• Deliver work products that are practical, concise, and ready for implementation.

Qualifications:

• Master's degree in Social Work (MSW) required.
• Active clinical or macro social work experience, preferably in healthcare, PACE, geriatrics, or reputed company care settings.
• Demonstrated experience translating social work practice into operational workflows, playbooks, or programs.
• Strong understanding of housing instability, community resources, and social determinants of health.
• Experience with care management models and performance or outcomes reporting.
• Excellent written communication skills, with the ability to create clear, actionable documentation.
• Comfortable working in a fast-moving, ambiguous environment with evolving priorities.

Compensation: We take into account an individual's qualifications, skillset, and experience in determining final salary. The expected salary range for this position is $58.00 -$67.00 hourly. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.
