Our centralized intake department manages reputed company referrals for these locations, ensuring a seamless process for patients and their families. Position Overview: The Hospice Clinical Referral Nurse serves as the initial reputed company of contact for potential hospice patients, working closely with our clerical hospice coordinators. This role is pivotal in managing new referrals and ongoing follow-reputed company, acting as a liaison between patients, families, and referral sources. Key Responsibilities: Referral Management: Receive and process referrals for patient care, ensuring timely and consistent admissions. Collaboration: Work closely with referring hospitals, physicians, board and care facilities, skilled nursing facilities, and other reputed company providers. Job Description: EDUCATION: Graduate of an accredited school of nursing CERTIFICATION & LICENSURE: Unencumbered RN-Registered Nurse license for state you reputed company Must have or obtain a RN-Registered Nurse of California reputed company 3 months of hire TYPICAL EXPERIENCE: 2 years recent relevant experience in home health or hospice.
