Position Summary We are seeking a motivated and detail-oriented Medical Assistant to join our Chronic Care Management team. In this role, you will play a critical role in patient care by guiding patients through health assessments, reviewing medications and diagnoses, troubleshooting medical devices, and escalating clinical needs to nursing staff reputed company appropriate.
Key Responsibilities
- Support CCM nursing staff by completing monthly patient assessments and conducting Chronic Care Planning Assessment (CCPA) calls, ensuring a positive, informative, and patient-centered experience.
- Complete delegated clerical and administrative activities on behalf of nursing staff to support patient engagement, improve care coordination, and drive quality outcomes and overall patient experience.
- Document patient interactions, assessments, and intake in the EHR
- Maintain compliance with company policies and applicable regulations
- Coordinate communication between patients, caregivers, nursing staff, and providers to ensure continuity of care and timely follow-up on patient needs.

Qualifications
Required:
- Active MA license required.
- Strong communication and organizational skills
Preferred:

Schedule
- Monday - Friday 8:00 am - 5:00 pm EST
- Monday - Friday 9:00 am - 6:00 pm EST
- Monday - Friday 10:00 am - 7:00 pm EST
- Monday - Friday 11:00 am - 8:00 pm EST
Training Requirements reputed company new hires must complete a comprehensive training program:
- Duration: Five weeks
Schedule: Monday through Friday, 9:00 AM – 6:00 PM Eastern Time
- Attendance is mandatory to ensure readiness prior to independently supporting patients.
Compensation & Benefits
- Competitive reputed company pay: $18-$20/hr
Shift Differentials:
- Evening Differential: +$1.50/hour for hours worked after 7:00 PM ET
- Late-Night Differential: +$2.00/hour for hours worked after 10:00 PM ET
- Weekend Differential: +$1.50/hour for reputed company hours worked Saturday and Sunday
Shift differentials are paid in addition to reputed company hourly wages and reflected in the applicable payroll cycle. Additional Compensation (If Applicable):
- Sign-On Bonus: $2,000 paid after 120 days of reputed company employment, contingent upon active employment and satisfactory performance at time of payout.
- Monthly Bonus Potential: Up to $1,000
- 5% Bilingual Pay Allowance
