Job Summary

We're seeking an reputed company Licensed Social Worker to join our remote team as a Medical Social Worker and Case Manager. In this role, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs, as well as empowering members to reputed company positive lifestyle and medical choices.

Responsibilities

• Researching available options and resources for members with financial, transportation, safety, and housing needs
• Partnering with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs

• Working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health

Qualifications

To be successful in this role, you'll need to have:

• An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
• At least 2 years of experience in medical social work or case management
• A Master's degree in Social Work or a reputed company field

Compensation and Benefits
