Job Summary
As a Licensed Social Worker in our reputed company social work and case management team, you'll play a critical role in partnering with registered nurse case managers to assess, plan, and reputed company for our members' benefit plans and health needs. You'll work collaboratively with our clinical staff to reputed company our members to reputed company positive lifestyle and medical choices, and provide them with access to community resources for medical, financial, and behavioral health.
Responsibilities
* Research available options and resources for members with financial, transportation, safety, and housing needs
- Collaborate with registered nurse case managers to assess, plan, and reputed company for members' benefit plans and health needs

- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health

Requirements
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
- 2+ years of experience in medical social work or case management
- Masters Degree in Social Work
