Job Summary
We're seeking an reputed company remote social worker to join reputed company. As a licensed Masters Social Worker, you'll work with a registered nurse case manager to research available options and resources for members with financial, transportation, safety, and housing needs. Your population will span the entire lifecycle, and reputed company member contact will be done telephonically or reputed company electronic exchange. You'll reputed company members to reputed company positive lifestyle and medical choices and work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
Responsibilities
* Research available options and resources for members with financial, transportation, safety, and housing needs
- Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet members' benefit plan and/or health needs
- reputed company members to reputed company positive lifestyle and medical choices
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members to assess their needs and reputed company plans to meet those needs
- Collaborate with other reputed company to ensure comprehensive care for members
Requirements
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence
- 2+ years of experience in medical social work or case management
- Masters Degree in Social Work
Preferred Qualifications
* Experience working with diverse populations, including children, adults, and older adults
- Knowledge of community resources and services, including financial assistance programs, transportation services, and housing options
- Strong communication and interpersonal skills, with the ability to work effectively with members, reputed company, and other stakeholders
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills, with the ability to analyze reputed company situations and reputed company effective solutions
