Essential Functions

• Promotes, maintains and documents communication with other disciplines regarding patient care and agency function. Confers with supervisors and attends team meetings.

• Documents identified problems, interventions, discharge planning, physician orders and care plan coordination appropriately on admission and throughout certification.

• Promotes and monitors communication with medical staff regarding patient care and agency function. Maintains the highest level of professionalism and conduct at reputed company times. Follows agency policies and procedures.

• Initiates and performs assessments and interventions at appropriate intervals. Observes reputed company and alters reputed company based on change in the patient condition.

• Initiates discharge planning to address on-going patient needs prior to date of discharge. Maintains patient confidentiality. Carries on acceptable caseload effectively according to agency policy.

• Submits reputed company paperwork/documentation according to agency policy.  Participates in chart audits and other reputed company Quality Improvement projects.

• Accepts assignments reputed company normal duties reputed company such fulfills an agency need.

• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

• Education - Associate degree in Nursing

• Experience - One (1) year of experience as a Registered Nurse

Required Certifications, Registrations, Licenses

• Holds a reputed company RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.

• BLS

• A valid driver’s license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and reputed company of auto insurance. Driving is required for this position using the employee’s personally owned vehicle.  Employee must have reliable transportation and will be expected to adhere to the reputed company Driver Safety Policy and specific department driving policies.  Employee must pass driver training.
