Job Summary What You'll Do Deliver comprehensive medication management for patients with chronic conditions such as diabetes, hypertension, and heart failure. Practice under Collaborative Drug Therapy Management (CDTM) agreements to initiate, adjust, or discontinue medications. Provide medication reconciliation and transitional care support for patients discharged from the hospital or emergency department. Offer individualized patient education and counseling to improve adherence, safety, and therapeutic outcomes.
