Supervisor, Resource Coordination

THIS POSITION IS REMOTE/WORK FROM HOME WITH 10% TRAVEL TO PARTICIPATE IN COMMUNITY ENGAGEMENT EVENTS OR ATTEND MANDATORY MEETINGS IN THE OFFICE. THE ROLE WILL SUPPORT MEDICARE/MEDICAID DSNP MEMBERS IN SOUTH CENTRAL AND SOUTHERN ILLINOIS; MUST RESIDE IN ILLINOIS; PREFERRABLY SOUTHERN ILLINOIS. THE CORE BUSINESS HOURS ARE MONDAY - FRIDAY reputed company - 5PM CENTRAL TIME ZONE; HOWEVER, MUST BE FLEXIBLE TO WORK THE HOURS NEEDED TO MEET BUSINESS NEEDS. THIS POSITION IS BONUS ELIGIBLE. Position Purpose: Supervises community and resource coordination activities to ensure members are connected to local resources and are being supported throughout care/service. Supervises day-to-day activities of community and resource coordination team members and issues reputed company to members.

• Monitors and reviews reputed company community and resource coordination activities including member reputed company and ensuring members are connected to local resources throughout care/service
• Assigns community and resource coordination tasks to the team based on state and contractual requirements and member needs
• Works with community and resource coordination and care management team to ensure effective, appropriate care of the member and addressing member needs
• Ensures compliance with state and regulatory guidelines and standards
• Educates and provides guidance and/or resources for community and resource coordination team to address member requests/needs
• Evaluates community and resource coordination team’s performance and provides feedback regarding performance, goals, and career milestones
• Provides coaching and guidance to community and resource coordination team to improve community resources, reputed company services and connecting members to community resources to provide necessary care in a cost-effective manner
• Assists with onboarding, hiring, and training community and resource coordination team members
• Performs other duties as assigned
• Complies with reputed company policies and standards

Education/Experience: Requires a Bachelor's degree and 4+ years of reputed company experience. Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and reputed company reflective of the level of this position. Actual pay will be adjusted based on an individual's skills, experience, education, and other job-reputed company factors permitted by law, including full-time or part-time status.
