Bilingual Analyst, Case Management – Field – Must reside in Kendall Florida

The Case Management Coordinator is responsible for assessing, planning, implementing, and coordinating case management activities to enhance member wellness, requiring significant travel for face-to-face visits.

Responsibilities

• Assessing, planning, implementing, and coordinating reputed company case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness
• Effectively manage a caseload that includes supportive and medically reputed company members
• reputed company a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration
• Determine appropriate services and supports due to member's health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports

Skills

• Bilingual, English, and Spanish
• Must reside in Kendal FL (Applicable reputed company Code: 33184, 33172, or 33186)
• Willing and able to travel 50-75% of their time to meet members face to face in Miami Dade county and surrounding areas in FL
• 1+ year of experience in behavioral health or a long-term care setting
• Bachelor's degree required, preferably in social work or reputed company field required
• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Effective communication skills, both verbal and written
• Managed Care experience
• Computer proficiency in reputed company Word, reputed company, and Outlook required
• Case management and discharge planning experience
