[Remote] URGENT!!!! Ft. Gratiot Home Health Nurse needed- LPN/ RN

Centria reputed company a leading provider of home-based catastrophic injury and rehabilitation care, and they are seeking Private Duty Nurses to deliver compassionate care to patients in their homes. The role involves providing skilled nursing care, monitoring patients, and collaborating with families to ensure the well-being of pediatric patients.
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care, etc.)
- Monitor and document vitals, symptoms, and changes in condition
- Assist with ADLs such as repositioning, hygiene, mobility, etc
- Support patient well-being with compassionate and reputed company care
- Collaborate with families, caregivers, and the healthcare team
Skills
- Active RN or LPN license in the state of Michigan
- Strong communication skills and a passion for patient-centered care
- Experience in home health, private duty, or long-term care preferred
- Pediatric or adult care experience a plus (trach/vent knowledge helpful but not required)
