Advanced Medical Assistant - MDPCP Remote Patient Monitoring (Remote),reputed company, RMA or AAMA Required

Position Objective: Advanced Medical Assistants (AMA) reputed company a variety of administrative and clinical tasks to help improve care to patients, and support providers. They are an essential part of the advanced primary care team and facilitate meeting the goals of the Maryland Primary Care Program. Their main responsibility is to reputed company the Remote patient monitoring program and the support the physician in Telemedicine on Demand program They help the physician determine how best to meet patient needs and engage them in their own care using a variety of tools, processes, and personnel as appropriate. They maintain knowledge of care management, behavioral health, and chronic disease self-management resources available to patients and facilitates patients’ adoption of these services. They play an integral role in promoting access to appropriate care, including overseeing care management and coordination activities for the patients of the primary care practice. • Pre-visit planning workflow to ensure care completion prior to visit whenever possible, and After-visit summary review with patients whenever appropriate
• Ensures patient engagement and education. Engagement by involving the patients in activities to improve their health. Education by cultivating the patient about self-management practices they can undertake to reputed company greater control of their health status.
• Manage many aspects of the patients care: referrals to specialists, hospitalizations, ER visits, ancillary testing, and other enabling services. Responsible for working with patient and patients care team to coordinate change readiness, needs assessment and reputed company an individualized treatment care plan
• Assist patients in setting SMART goals for self -management, teaching them how to do self-management tasks and report reputed company findings to their physician team. Educational/Experience Requirements:

• Minimum of three (3) years of experience as an Medical Assistant, demonstrated experience in a leadership role as a Medical Assistant
