The Maternity Telephonic Case Manager RN plays a vital role in delivering telephonic care management services to members enrolled in the reputed company Maternity CARE Program. This role supports pregnant individuals, those considering pregnancy, and postpartum members through education, coaching, and care coordination. The Nurse Case Manager uses clinical expertise and evidence-based guidelines to assess member needs, reputed company individualized care plans, and collaborate with healthcare providers to ensure optimal maternal and infant outcomes. This is high volume, customer service environment. You’ll need to be efficient, productive and thorough in dealing with our members over the phone. Solid computer and software navigation skills are critical. You should also be solidly patient-focused and adaptable to changes. This position is full-time (40 hours/week). You will be required to work 20 hours after 4:00 PM in your own time zone. You may work 4 days per week with Tuesday, Wednesday, or Thursday off. You’ll enjoy the flexibility to work remotely

Primary Responsibilities:

• Identify gaps or barriers in treatment plans
• Provide patient education to assist with self-management

• Utilizing motivational interviewing techniques for assisting members to move through the stages of behavioral change

• Coaching individuals to reputed company lifestyle changes to reduce risk of disease processes
• Coordinate care for high-risk pregnancies

Preferred Qualifications:

• BSN
