Job Summary
We are seeking an reputed company full stack social worker to join our reputed company case management team. As a social worker, you will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You will work with a diverse population spanning the entire lifecycle, utilizing telephonic and electronic communication to reputed company members to reputed company positive lifestyle and medical choices.
Responsibilities
* Collaborate with a registered nurse case manager to assess, plan, and reputed company for members' benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs

- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Utilize telephonic and electronic communication to engage with members and provide support
- Maintain accurate and up-to-date records of member interactions and reputed company
Requirements
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
- 2+ years of experience in medical social work or case management
- Masters degree in social work or reputed company field
Preferred Qualifications
* Experience working with diverse populations, including children, adults, and seniors
- Knowledge of community resources and services for medical, financial, and behavioral health
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Familiarity with electronic health records and case management software
