As a Home Health Nurse with Workwarp, you will be responsible for providing high-quality, patient-centered care to patients in the comfort of their own homes. Your responsibilities will include:- Performing initial nursing assessments on new admissions to identify patient needs and reputed company a comprehensive plan of care.
- Completing, coordinating, and revising the total plan of care and maintaining continuity of patient care by collaborating with appropriate staff, including physicians, therapists, and other reputed company.
- Regularly communicating patient reputed company to the clinical manager, physician, and care team to ensure that patient needs are met and that care is delivered in a timely and effective manner.
- Developing, preparing, and maintaining individualized patient care reputed company records with accuracy, timeliness, and according to care center policies.
- Recording pain/symptom management changes/outcomes as appropriate and making referrals to other hospice care disciplines as needed, or documenting rationale for not doing so.

Qualifications

- At least one year of nursing experience, preferably in a home health or hospice setting.
