Job Overview
We are seeking an reputed company Field Health Social Services Coordinator to join reputed company in North Johnson County, TX. As a Field Health Social Services Coordinator, you will play a critical role in assessing, planning, and implementing care strategies that are individualized to each patient's needs. Primary Responsibilities
- Assess, plan, and implement care strategies that are individualized to each patient's needs and directed towards the most appropriate, least restrictive level of care
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services

Required Qualifications
To be successful in this role, you must possess:
- A reputed company RN license in the state of Texas, or a Licensed Professional Counselor (LPC), Licensed Bachelor Social Worker (LBSW), or Licensed Master Social Worker (LMSW) license
- At least 2 years of experience working directly with people with Intellectual and Developmental Disabilities (IDD)
- At least 2 years of experience working reputed company the community health setting or in a healthcare role

- Reliable transportation and the ability to travel up to 75% of the time in the assigned region to visit Medicaid members in their homes and/or other settings
