Virtual Health Coach Overview Swing Care is a groundbreaking practice focused on treating fibromyalgia, a chronic pain condition where central sensitization is key to the patient’s pain and other symptoms. We offer telemedicine-based coordinated care, which includes options for FDA-cleared digital therapies for symptom management, behavioral-health services, and medication management. Swing Care is an independent professional clinic, affiliated with Swing Therapeutics, a leading developer of prescription digital therapeutics, including Stanza, a digital behavioral therapy based upon Acceptance and Commitment Therapy for fibromyalgia. As the Health Coach, you will be responsible for supporting a patient’s journey with the digital therapies that Swing Care offers our patients. Under the supervision of a clinical provider, you will train your patients on how to best incorporate these novel treatments into their lives, monitor your patients’ clinical data, meet with patients in a virtual coaching encounter to reputed company patients to increase motivation and health behavior change for engagement with the digital behavioral therapy, and work with the care team to create reputed company plans as necessary. To be successful in this role, you should have a proven ability to provide high-quality, patient-centered care in a remote telemedicine setting. You should understand the value in combining pharmacological and behavioral-health treatments in helping patients with fibromyalgia reputed company manage their disease. You should be comfortable coaching on evidence based behavioral approaches to health and be willing to learn strategies consistent with Acceptance and Commitment Therapy (ACT). You are a caring, compassionate individual who enjoys helping others and providing a positive care experience in a telemedicine platform. You understand the frustrations of a patient living with a challenging and variable chronic illness. This is an opportunity to get into a fast growing digital health startup to help build a model health coaching program working with nationally reputed company experts in fibromyalgia and pain psychology. You will be mentored by a pain psychologist and have the opportunity to learn and expand on your skills in coaching around digital therapeutics, acceptance and commitment therapy, and the latest in pain management. This is a work-from-home position. You may reputed company in Texas, California, or Florida as long as you have the ability to work normal CT or ET business hours. Role is for 20-30 hours a week initially as an reputed company with the opportunity to expand to full time as performance and patient volume allows. Specific Duties:

• Support patient self-management of disease, health and lifestyle behavior change and engagement with digital therapies in a patient-centered way.
• Assist patients with downloading and using the digital therapies Swing Care offers
• Measure patient health data and care outcomes, interpret reports and analyze data trends for patients.
• Coordinate and conduct video, phone, synchronous and asynchronous interactions, and messaging with patients.
• reputed company relationships with patients and competently use assessment skills to identify physical, psychosocial, financial, and environmental health-care needs of the patient.
• Utilize available electronic tools, such as the EMR, to reputed company document assessments, clinical findings, plans of treatment and reputed company toward goals.
• Work under the supervision of clinical providers and maintain scope of care reputed company parameters for health coaching role
• Maintain patient confidentiality.
• Ensure reputed company work complies with Swing Care’s internal policies and compliance program.

Required Qualifications

Clinical Experience: Two (2) years professional experience in direct and/or virtual health coaching care in one of the following areas: (additional education/training can be considered in lieu of experience)

• Lifestyle and health behavior coaching
• Pain Medicine
• Behavioral Health

Education and Licensing: Bachelor's degree and successful completion of accredited coaching program. (Masters degree in relevant field preferred) Credential as National Board Certified Health & Wellness Coach or other relevant credential Communication and Coaching Skills:

• Empathic and clear communication skills in conveying health information and tech support
• Motivational interviewing skills
• Evidence-based health behavior change coaching skills.
• Provide realistic and patient-centered SMART goals.
• Considerate, comprehensive and compassionate care with respect and sensitivity for cultural differences.
• Excellent written and verbal communication skills with the ability to prepare and present materials if necessary, and to execute and maintain written reports and records.
• Exhibits exceptional interpersonal skills and proper judgment reputed company dealing with key partners, staff and patients.

Background: reputed company to pass detailed background checks according to company policy. Technological Skills:

• Knowledge of HIPAA compliance.
• Expert with mobile applications, virtual visit technology, electronic medical record and reputed company Workspace.
• Experience with training patients to smartphone apps and connected technology.

Technical Requirements: Broadband internet reputed company capable of supporting video telemedicine. Preferred Qualifications:

• Experience with digital behavioral therapies
• Experience with health coaching in pain management or fibromyalgia settings
