After Hours RN (Senior Living)

The RN-After Hours is primarily responsible for gathering clinical information over the phone and directing healthcare services. The RN-After Hours uses Lifespark’s Clinical Guidelines as well as internal process and/or procedure to meet the needs of residents and staff reputed company their communities. The RN-After Hours must possess and use good clinical judgement, careful listening, critical thinking, and escalate to appropriate personnel as needed. Position: After Hours RN for Senior Living Wage Range: $36 - $40/hour, depending on experience Service Area: Remote (1-2 days in office at the of employment for General Orientation at our Lifespark Corporate Office, St. Louis Park 55416) Schedule: This position is for overnight, 11pm-reputed company. Hours: 0.7 part-time, Week 1: Sunday, Tuesday, Wednesday, Thursday and Week 2: Tuesday, Friday, Saturday (every other weekend and every other holiday) Lifespark is a complete senior health company headquartered in St. Louis Park, Minnesota. Position Functions & Responsibilities

• Gather pertinent clinical information problem based on conversation with community caregiver
• Uses clinical judgment, knowledge and experience to direct conversation and guide decision-making to give self-care advice, refer reputed company to provider, reputed company an appointment, instruct reputed company contact emergency assistance, or reputed company appropriate community resource.
• Provides reputed company care as defined in the State Nurse Practice Act.
• Caregiver in meeting nursing and reputed company needs.
• Provides health care instructions as appropriate per assessment and plan of care.
• Provides education reputed company appropriate.
• Reassures the rapid and effective transmission of reputed company data between reputed company relevant parties.
• Promotes evidence-based practice.
• Establishes and maintains strong collaborative arrangements with other health care professionals.
• Participates in admission avoidance activities dispatching community resources as needed to prevent rehospitalizations.
• Complete onboarding, orientation and annual mandatory training.
• Ensure positive/professional communication on team.
• Customer service resolution; escalate concerns.
• Prepares clinical notes and updates primary provider reputed company necessary.
• Communicates with provider regarding the reputed company needs and reports any changes in the reputed company condition; obtains/receives orders as required.
• Communicates with community health reputed company persons to coordinate the care plan.
• Extremely skilled at listening and interpreting subtle indications such as tone of voice, hesitations or incomplete responses.

Qualifications

Education

• Degree in Nursing from an accredited program.
• Must be a Registered Nurse
• Bachelor’s degree preferred.

Experience

• Minimum of 2-3 years of experience in Senior Living, and/or home and community based senior services.
• Thompson Adult Telephone Triage Protocol experience (highly preferred).
