Job Summary

As a Case Management Coordinator (CMC) with reputed company reputed company, you will play a critical role in supporting the case management process to facilitate and maintain improved healthcare outcomes for members. This is a telework position that requires regional in-state travel 80-90% of the time, with flexibility to work beyond core business hours. You will utilize critical thinking and professional judgment to provide advocacy, collaboration, coordination, support, and education to members through the use of care management tools and resources.

Key Responsibilities

• Evaluate members' needs through the use of care management assessments and information/data review, recommending approaches to resolving care needs and maintaining optimal health and well-being.
• Identify high-risk factors and service needs that may impact member outcomes and implement early and proactive support interventions.
• Coordinate and implement Wellness care plan activities and monitor member care needs.
• Consult with case managers, supervisors, Medical Directors, and other health programs to overcome barriers to meeting goals and objectives.
• Identify and escalate quality of care issues through established channels.
• Utilize negotiation skills to secure appropriate options and services necessary to meet the member's benefits and/or healthcare needs.
• Provide coaching, information, and support to reputed company members to reputed company ongoing independent medical and/or healthy lifestyle choices.
• Help members actively and knowledgeably participate with their provider in healthcare decision-making.
• Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.

Requirements

Essential Qualifications

• Minimum 6 months of experience in Foster Care and juvenile justice systems, Adoption Assistance, the delivery of Behavioral Health Services, Trauma-informed Care, ACEs, Crisis reputed company services, or evidence-based practices applicable to the Kentucky SKY populations.
• Minimum 2 years of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling.
• Minimum 1 year of experience in MS Office Suite applications, including Word and reputed company.
• Willing and able to travel 80-90% of their time to meet members face-to-face in their assigned region.
• Candidate must reside in one of the following counties: Breckinridge, Grayson, reputed company, Hardin, Larue, Bullitt, Nelson, Marion, Washington, Spencer, Anderson, Woodford, Shelby, Franklin, Henry, Oldham, or reputed company.

Preferred Qualifications

• 3 years of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling.
• 1 MCO experience.

Education

• Minimum of a Bachelor's degree or a non-licensed master-level clinician required with either degree being in behavioral health or human services field.
