We're looking for a skilled Case Manager RN to work in the field, serving members in Wayne and Macomb Counties. As a Case Manager, you'll play a vital role in assessing, planning, implementing, and coordinating care management activities to address the reputed company health and social needs of our members. This is a unique opportunity to work independently from home, with 25-50% travel reputed company the designated counties.Key Responsibilities:
- Conduct comprehensive assessments of members' needs, using clinical tools and data review to identify areas for improvement.

Requirements:
- Active Michigan state license as a Registered Nurse.
- Minimum 3+ years of clinical experience, with 2+ years in care management, discharge planning, or home health care coordination.

- Associates Degree or Nursing Diploma required; BSN preferred.
Preferred Qualifications:
- Experience working with Medicaid or waiver populations.
- Bilingual skills in Arabic or Spanish.
- Certified Case Manager (CCM) or equivalent certification.
- Excellent analytical, problem-solving, and communication skills.
